'Outstanding Service to the Viral Hepatitis Sector' Award

This award will be presented to an individual who has displayed outstanding service to the viral hepatitis sector.
Award Eligibility
Individuals are eligible to receive the ‘Outstanding Service to the Viral Hepatitis Health Promotion Sector’ award if:
- The individual has made a significant contribution in the area of viral hepatitis health promotion.
- The individual demonstrates respect, commitment and collaboration
- They are currently employed by, or a volunteer for, a government or non-government organisation with a significant focus of their role on viral hepatitis health promotion
How to make a nomination
‘Outstanding Service to the Viral Hepatitis Health Promotion Sector’ - Nominations can be made by an individual’s organisation. Nominations will not be accepted from the individual nominated.
To make a nomination you need to complete the awards Nomination form. This requires you to provide:
- Your contact details
- The name and contact details of the individual you are nominating
- A brief biography of the nominee’s roles in the viral hepatitis sector
- A brief overview of the key achievements and/or key activities of the nominee in the viral hepatitis sector
- A brief statement outlining how the nominees have demonstrated commitment, respect and collaboration
- Any additional information that demonstrates how the nominee has excelled against any of the judging criteria listed above.
All nominations must be received by COB 26th September 2009.
Judging Criteria
The awards will be judged by a panel of three judges representing peak not-for-profit community organisations.
In judging the ‘Outstanding Service to the Viral Hepatitis Health Promotion Sector’ award the following key criteria will be assessed.
Commitment
- The individual has demonstrated a commitment to viral hepatitis health promotion that has gone beyond that required to achieve the objectives of their role.
- The individual has continually demonstrated a commitment to effective practice in their role.
Respect and collaboration
- The individual continually demonstrates respect for people living with, or at risk of, viral hepatitis.
- The individual continually employs a collaborative approach to engage members of target groups and/or partner organisations in planning and implementation of projects/programs or services.
Winners will be announced at the National Hepatitis Health Promotion Conference Dinner on the 29th of October, 2009.
