‘Outstanding Service to Viral Hepatitis Health Promotion’

This award will be presented to an individual who has displayed outstanding service to the viral hepatitis sector.

Award Eligibility
Individuals are eligible to receive the ‘Outstanding Service to Viral Hepatitis Health Promotion ’ award if:
  • The individual has made a significant contribution in the area of viral hepatitis health promotion.
  • The individual demonstrates respect, commitment and collaboration.
  • They are currently employed by, or a volunteer for, a government or non-government organisation with a significant focus of their role on viral hepatitis health promotion.

How to make a nomination
‘Outstanding Service to Viral Hepatitis Health Promotion ’ - Nominations can be made by any organisation or professional group including the nominee’s organisation. Nominations must be endorsed by the CEO or President of the organisation. Nominations from individuals nominated for the award will not be accepted. To make a nomination you need to complete the awards nomination form. This requires you to provide:
  • Your email and day-time phone contact details
  • The name and contact details of the individual you are nominating
  • A brief biography of the nominee’s roles in the viral hepatitis sector
  • A brief overview of the key achievements and/or key activities of the nominee in the viral hepatitis sector
  • A brief statement outlining how the nominees have demonstrated commitment, respect and collaboration
  • Any additional information that demonstrates how the nominee has excelled against any of the judging criteria listed above.

All nominations must be received by COB 4th of November, 2011.


Judging Criteria

The awards will be judged by a panel of three judges representing peak not-for-profit community organisations.

In judging the ‘Outstanding Service to Viral Hepatitis Health Promotion ’ award the following key criteria will be assessed.

Commitment

  • The individual has demonstrated a commitment to viral hepatitis health promotion that has gone beyond that required to achieve the objectives of their role.
Respect and collaboration
  • The individual continually demonstrates respect for people living with, or at risk of, viral hepatitis.
  • The individual continually employs a collaborative approach to engage members of target groups and/or partner organisations in planning and implementation of projects/programs or services.

Winners will be announced at the National Hepatitis Health Promotion Conference Dinner on the 24th November, 2011.
